Form Description:
Employee pulse forms were developed to help us measure engagement and performance while holding each other accountable to our core values.
This brief survey allows you to provide constructive feedback or encouragement about a colleague in a confidential and private manner. All submissions are sent directly to Human Resources, where they are reviewed, and appropriate actions are taken based on HR’s evaluation of the situation.
Employee Change of Information Form

Form Description:
Employee change of info. should be filled out if any of the following changes are made while employed at Lightning Electric.
-Change of Address
-Change of Phone #
-Change of First or Last Name
-Change of Deposit or routing info. for Direct Deposit
Any of the above changes need submitted to HR to update your employee profile.
Career Checkup Form

Form Description:
Each year L.E.I. employee’s are required to complete a Career Checkup review. This is a place for you to record and communicate your interest and a planning aid to help you match your individual capabilities and interests with Lighting's needs. Actual progress and ability to achieve this plan is dependent on both your personal development and Lightning Electric's needs.
· This form will be required to be completed by Human Resources prior to your scheduled review date.
· If you have previously completed a form you will receive a copy of your last Career checkup entry to review your successes and challenges since your last Career Checkup.
